PPO Insurance Support

Elite members pay their discounted rate at the time of treatment. We prepare and submit out-of-network PPO claims with full documentation. When reimbursement arrives, we send those funds directly back to you. Routine cleanings and your Oral Health Assessment are covered through your plan and are not submitted to insurance.

Activation Works

A clear path to enrollment.

Comprehensive Oral Assessment

Your assessment is $185 and includes a full clinical exam and complete X-rays to establish your baseline and determine readiness for prevention-based care.

Clinical Review and Planning

You meet with your doctor, review findings, and outline the best path forward.

Immediate Plan Activation

If you are a good candidate, your plan activates before you leave. Your first cleaning credit is ready immediately.

The Elite Protection Plan is designed for prevention and maintenance. If your assessment reveals that treatment is needed before preventive care can begin, those needs are addressed first. Once your oral health is stable, you can join the plan.
